Day 1

Patients are required to surrender their medications, passport, and cash for safekeeping. Clinic staff will check your belongings for any drugs or dangerous items. You are welcome to keep your electronic devices. To minimize travel stress, we recommend arriving on a morning or early afternoon flight.

Days 2–3

Your safety is our top priority. An EKG will be performed, along with blood and urine tests for a comprehensive health evaluation. Our in-house cardiologist will conduct an ultrasound of your heart.

Our therapists and support staff will help establish your intentions and post-treatment goals. You will participate in yoga, stretching, and massages to prepare mentally, physically, and spiritually for your treatment day.

Day 4

Treatment usually begins at 9:00 A.M. in a comfortable, calming setting. Our expert nurses will start your IV, connect you to a heart monitor, and administer Dramamine to mitigate nausea. Ibogaine HCl is given in capsule form on an empty stomach with dosage tailored to your weight.

Approximately one hour after dosing you may feel tingling, warmth, and auditory sensations, followed by vivid visions lasting 3–5 hours. Once visions subside you enter a deeply introspective processing stage. You will remain in the treatment room for 8 hours post-dosing.

Day 5

You will have an integration session with one of our expert psychotherapists to derive meaningful insights from your experience in alignment with your goals. We will also begin shaping your follow-up aftercare plan.

Transcend's medical team will assess your EKG and other parameters to determine if a maintenance (booster) dose is appropriate. A fractional dose may be given to further promote neuroplasticity — it does not induce visions and requires only 2 hours of monitoring.

Day 6

Continued integration sessions with our therapists, focused on consolidating the insights from your treatment and finalizing your aftercare plan. Many patients report a renewed sense of improvement each day following the flood dose.

Our team will ensure all your needs are addressed and that you leave with a solid, reality-based plan for sustained recovery.

Day 7

Your final morning at the clinic. Staff will assist with discharge planning, answer any remaining questions, and coordinate your private transfer back to Cancun International Airport (CUN) for your flight home.

Aftercare

Aftercare is a critical part of the ibogaine treatment process and varies for each patient. Typically it involves regular contact with an aftercare specialist for several months post-treatment.

Duration: 3–12 months depending on condition. Goal: Convert the neuroplasticity phase into durable behavioral change, emotional regulation, and long-term stability. Transcend's approach to aftercare is tailored to each patient's unique journey.
